Neuropathic Pain (NP): Overview, Causes, Symptoms, and Management
Neuropathic pain (NP) is a complex, chronic pain condition that arises from damage or disease affecting the somatosensory nervous system, which includes peripheral nerves, the spinal cord, and the brain128. Unlike nociceptive pain, which results from tissue injury, neuropathic pain is due to abnormal processing of pain signals by the nervous system itself.
Causes
Neuropathic pain can be triggered by a wide range of conditions and injuries, including:
- Metabolic disorders: Such as diabetic peripheral neuropathy.
- Viral infections: For example, postherpetic neuralgia (after shingles) and HIV.
- Autoimmune diseases: Including multiple sclerosis and Guillain–Barre syndrome.
- Chemotherapy-induced neuropathies: From certain cancer treatments.
- Trauma: Such as spinal cord injury, nerve compression (e.g., herniated disc, carpal tunnel syndrome), or amputation (phantom limb pain).
- Inflammatory and hereditary neuropathies.

Symptoms
Common symptoms of neuropathic pain include:
- Burning, tingling, or shooting pain
- Allodynia: Pain from normally non-painful stimuli, such as light touch or cold.
- Hyperalgesia: Increased sensitivity to painful stimuli.
- Deep, aching pain
- Numbness or loss of sensation
- Muscle weakness or wasting (in chronic cases)
- Pain persisting after the initial cause has resolved, due to changes in nervous system sensitivity2367.
Pathophysiology
Neuropathic pain results from maladaptive changes in the nervous system, including:
- Peripheral and central sensitization: Heightened responsiveness of nerve fibers and central pathways.
- Ectopic activity: Abnormal firing in damaged nerves or dorsal root ganglia.
- Altered ion channel function, immune cell activation, and inflammatory mediator release.
- Structural changes: Such as nerve sprouting and loss of inhibitory control in pain pathways68.
Diagnosis
Diagnosis is primarily clinical, based on history and characteristic symptoms. It may be supported by:

Management
Neuropathic pain is often persistent and challenging to treat. A multidisciplinary approach is recommended:
- First-line medications: Tricyclic antidepressants (TCAs), serotonin-norepinephrine reuptake inhibitors (SNRIs), gabapentinoids (gabapentin, pregabalin), topical agents (lidocaine, capsaicin)47.
- Second-line options: Combination therapy, tramadol, tapentadol.
- Third-line and beyond: Other antidepressants, anticonvulsants, NMDA antagonists, interventional therapies, neurostimulation, and, rarely, low-dose opioids4.
- Non-pharmacological therapies: Physical therapy, psychological support (CBT), exercise, and multidisciplinary pain management4.
- Treatment is individualized based on the cause, severity, and patient response5.
